Delta Health and Wellness Center is a nonprofit community clinic that provides primary care services. DHWC is an outpatient medical clinic that serves uninsured, underinsured and patients with Medicaid and Medicare insurance. The mission of DHWC is to provide increased access to basic medical needs, to provide a medical home, and to serve the community with quality primary medical care.
The DHWC is committed to improving the health of the residents within the county and is passionate about collaborating with community agencies to increase referrals to primary medical care services. The DHWC vision includes being the gateway health care clinic for primary medical needs by providing a continuum of services and referrals that give patients an empowered ability to care for themselves and those around them. Our experienced Nurse Practitioner provides a range of health care services for ALL ages of people 0-101!
We have a reduced fee and easy office hours, making your health visit an important appointment in your day! Office hours are Tuesdays and Fridays 2-6 p.m. and Saturdays 10 a.m. – 2 p.m. We request that you call for an appointment, even for same day appointments, so we know that you are coming in!
We have a sliding-fee scale available for families and individuals that qualify. All health appointments are $60.
Our family nurse practitioner can provide a wide range of services, including immunizations and well child services, sports physicals, prescription management, prenatal and newborn services, well woman care, referrals to other health related professionals and specialists, ordering of labs and X-rays, patient education, recycling of durable medical equipment and others.
As health care changes, the staff and Board at DHWC is committed to continuing to make primary care affordable and accessible to EVERYONE, no matter the insurance or size of pocketbook.
There are clinic visit vouchers for sale so that you can give the gift of health to anyone! We accept donations of new, unused medical supplies and of course welcome all monetary donations to our nonprofit clinic!
The clinic is governed by a volunteer Board of Directors, which involves community stakeholders. The board’s primary job is to create and continue the DHWC’s presence in the community, through a unified and dynamic mission and vision, along with the fundraising and grant writing needed to maintain the operating budget.
Since DHWC is a newly formed clinic, the board is working on soliciting grant funds to maintain and expand the services offered. We welcome all donations and are able to give a tax receipt in return for your donation this fantastic nonprofit.
